Hospice focuses on comfort care for Heart Disease patients rather than curative treatment. Our patients’ wishes are most important. Our team will work together to create an individualized plan for the patients’ goals of care to keep symptoms managed. The interdisciplinary care team includes our full-time, on-staff Medical Director, physicians, registered nurses, social workers, hospice aides, bereavement counselors and volunteers.
